Health Care Powers of Attorney Everyone should have a Health Care Power of Attorney. This is a legal document, similar to a living will, in which you name someone you trust to make decisions about your health care should you become incapacitated. For your convenience, we have provided links to the Health Care Power of Attorney forms for Florida, South Carolina, and the District of Columbia. These forms include detailed instructions for fulfilling statutory requirements in order to ensure the legality of the Power of Attorney. Follow the directions carefully! The following documents are in PDF format. Most newer computers include the Adobe Acrobat Reader that enables you to view PDF documents. However, if you do not have Acrobat, you may download it free from the Adobe website. (The Adobe site will open in a new browser window. After you have downloaded the program, close that window to return to this site.)
